Did You Know…

The transfer of patient identifiers, such as name, date of birth, hospital numbers, is a breach of privacy and confidentiality laws, and are reportable incidences to your REB/IRB or Privacy Office?

To avoid this breach from occurring, don’t forget to remove identifiable patient information from all data sent to the EMPOWER CC. Be sure to check copies of documents such as ECGs and to use substitute dates of birth on forms.

A substitute date of birth will include the patient’s month and year of birth, but will not include their actual date of birth.  Dates of birth should be recorded as follows:

ACTUAL DATE OF BIRTH SUBSTITUTE DATE
Patients born on the 1st -15th 1st
Patients born on the 16th   - last day of the month 15th
